What is Rhinoplasty? Rhinoplasty (nose surgery), is a procedure to improve the shape, angle and size of your nose, and to improve its balance and harmony with your face.  This can be accomplished by:

  • increasing or decreasing nasal size,

  • shaping the nasal tip,

  • narrowing the width of your nostrils, and

  • increasing or decreasing the angle between your nose and upper lip.  

 

The procedure can be performed in either an open or closed fashion.  An open rhinoplasty includes a small incision in the skin on the underside of your nostrils, which connects with other incisions inside your nose.  This small scar is barely visible once it heals.  

During a closed rhinoplasty, all incisions are made inside the nose.   Once access is gained to the cartilage and bones that support the nose, adjustments can be made to these structures to produce the desired changes.  

Rhinoplasty can also be performed to improve obstructed airways. This is accomplished through correction of a deviated septum, or correction of lateral nasal, bony or cartilaginous structures, to improve alignment and breathing.

Is Rhinoplasty Surgery Right For Me?

The best candidates for Rhinoplasty are otherwise healthy adults, who are nonsmokers, who are unhappy with the appearance of their nose, or who have breathing problems because of deviation of their airways within the nose.  

Some of the most common complaints patients present with include feeling that your nose is too large, too small, too wide, too narrow, or not in proportion to your face.  Other complaints include a prominent nasal hump, and an elevated or drooping nasal tip.  

Your surgeon can help determine if rhinoplasty is right for you, after careful examination of your internal and external nasal anatomy and skin quality, and consideration of your individual goals.  Results of nasal surgery are enhanced by concurrently assessing facial proportions including the projection of your chin to help achieve optimal facial balance.   

Your results may also be improved by improving the health, color and texture of your facial skin through combined treatment with non-surgical facial rejuvenation therapies including skin care systems, injectable fillers, toxin therapy, and laser resurfacing.

Length of Surgery:

Rhinoplasty usually takes between 3 and 4 hours depending on your internal and external nasal anatomy, and the extent of correction needed.  This surgery is performed in a hospital under general anesthesia and additionally using a local anesthetic.   You will likely be discharged home the day of surgery.

Expected Recovery:

Most patients are able to return to work within about 10 days.  You may begin light exercise at 2 weeks, while 6 weeks is required, before returning to strenuous activity or lifting.  Bruising may be present for up to 3 weeks.  Because of swelling and healing tissues, the final appearance of your nose may not be evident until one year after surgery.  Patients are instructed to protect themselves from direct sun exposure for 1 year, and to wear sunscreen.   Silicone therapy is additionally used to improve appearance of scars.

Common Side Effects:

Bruising and swelling around the eyes, headaches and facial aching, nasal drainage, discomfort from nasal packing.

Surgical Risks:

Risks include bleeding, infection, hematoma, changes in skin sensation, scarring, damage to underlying structures, asymmetry, unsatisfactory results requiring additional surgery, breathing difficulty.

Can expressing a pimple 13 weeks post Rhinoplasty affect the aesthetic result?

Cysts occur commonly in the skin, and will typically resolve once they are appropriately drained.  What you are palpating that disappears, may be a small fluid collection.However, without an in person examination, it is impossible to tell whether this particular lesion will lead to any scarring, or whether it is a sign of something related to your surgery.  The other question that has to be answered relative to what you are experiencing, before an outcome can be determined, is whether your surgeon used any grafts or foreign material to complete your surgery.   I would contact your surgeon so that he or she can examine you thoroughly and determine whether any additional management is necessary. It is best to always call the office before you do or use anything post operatively that Dr. Capuano has not recommended or prescribed for post and pre operative management. 

What is the cost of a Primary Cosmetic Rhinoplasty?

The cost of a Primary Cosmetic Rhinoplasty can vary significantly, though if it is simply that diagnosis the cost should be around $6000.

Would I benefit from a rhinoplasty? If so, do you recommend an open or closed rhinoplasty?

Thank you for your question.   After review of your photographs and your goals of surgery, I believe you would benefit from a rhinoplasty that refines your tip and allows it to blend smoothly with your upper nasal sidewalls.  Although this procedure can be performed by either an open or closed approach, the open approach allows your surgeon to see the structures that make your nose the shape that it is.  This allows your surgeon to be more precise which I feel in your case may result in a better overall outcome.  The incision made in an open approach only differs from that of the closed approach by a 4 to 5 millimeter incision across the narrowest portion of the columella and this incision tends to heal very well.  An important first step is to schedule an in-person consultation with Dr. Capuano, who is a Board Certified Plastic Surgeon and is experienced in rhinoplasty and comfortable with both approaches.  Hope this helps.

Hi I’m 8 weeks post op from a septo-rhinoplasty by an ENT w/hump removal & still have some swelling. I've noticed that after most of the swelling it looks like I have an inverted V. Having trouble breathing fully like there’s something stuck inside?

Hello and thank you for your question.  Because you are still 8 weeks post-op, there may be some remaining edema in your dorsum and tip, and this can obscure evidence of underlying bony or cartilagenous deformity in photographs until it resolves.  After viewing your photograph however, it does not appear that you have any gross evidence of development of an inverted V deformity or collapse of your upper or midvault.  Lateral or oblique photo's would be helpful, but an in person examination can provide your surgeon with much more information.  If you do eventually develop a collapse or inverted v deformity, you would not want to operate on it so soon after your initial surgery, so the best think to do at this point is to wait and see how it remodels.  Hope this helps.

How can I improve my profile?

Thank you for the question, cased on your photographs you may benefit from a rhinoplasty with a dorsal hump reduction and possibly some tip refinement as well as a concurrent chin implant placement.  Achieving facial harmony on both the frontal view as well as profile views is the ultimate goal of this surgical procedure.
